The first edition of this book was written by single author. The second edition(1996) was written by two authors. The third edition is written by several authors. The third edition is designed to be the reference textbook in the field. It incorporates all relevant technical and clinical data. Each chapter is written by a leading authority in that particular subject. The chapters were carefully reviewed and edited for consistency and accuracy. There is major emphasis on incorporating tables and figures to allow using the book as a teaching resource. It is written with not only the specialist in mind but also the general reader as each chapter stands on its own. To our knowledge, there is no other textbook in the field that field that comes close to this one in terms of content, scholarship and style.
